Certified Nursing Assistant (CNA)

The Certified Nursing Assistant is a member of the home care team trained to provide personal care, other unskilled services, and companionship in the home setting, under the direction, instruction, and supervision of a Registered Nurse, therapist, and/or clinical management and the patient. Performs various personal care services as necessary to meet the patient's personal needs and to promote the patient's comfort and a safe environment.

Home Health Aide (HHA)

The Home Health Aide is a member of the home care team trained to provide personal care, other unskilled services, and companionship in the home setting, under the direction, instruction, and supervision of a Director of Nursing/Registered Nurse designee and the patient.

Licensed Practical Nurse (LPN)

Under supervision of a registered nurse, provides quality care to patients and performs technical skilled care in patient’s home in compliance with nursing standards, federal, state, local government regulations and company clinical policies.

Registered Nurse (RN)

Under general supervision of clinical management, provides skilled nursing care to assigned patients and directs Licensed Practical Nurses and Home Health Aides to provide quality care. Assesses, plans, implements, evaluates total patient care and coordinates patient/family teaching. Assures compliance with nursing standards, federal, state, and local government regulations, and company clinical policies. Provides goal directed care leading to specific outcomes. May be on call for after-hours work.

Office Manager/Assistant Administrator

The incumbent is responsible for the overall management of a facility. Plans, develops, directs, monitors and supports all operational, administrative, clinical, human resources, customer service and fiscal activities for the Agency’s programs and services.
